Trientine (Cuvrior) is a copper chelator primarily used for the treatment of adult patients with stable Wilson's disease who have achieved copper excretion and are tolerant to penicillamine. Its main component is trientine tetrahydrochloride, which promotes the excretion of copper ions in the body through chelation. Although its efficacy is well-established, side effects may occur during use.

What Are the Side Effects of Trientine (Curior)?

Gastrointestinal Reactions

Symptoms: Abdominal pain (19%), changes in bowel habits (15%), etc.

Relief Measures: Take the medication on an empty stomach (1 hour before a meal or 2 hours after a meal) to reduce food irritation.

Divide the daily dosage into separate administrations (twice a day, morning and evening) to avoid excessively high single doses.

Skin and Hair Abnormalities

Symptoms: Rash (12%), hair loss (8%).

Relief Measures: Mild rashes can be monitored; if they persist or worsen, discontinue the medication and consult a doctor.

Hair loss is mostly temporary, and supplementing with protein and vitamins may help improve the condition.

Mood Swings and Anemia

Symptoms: Mood swings (8%), occasional anemia (4%).

Relief Measures: Monitor mental health and seek psychological support if necessary.

Patients with anemia need to have their iron stores evaluated; if iron supplements are required, they should be taken at least 2 hours apart from trientine.

Severe Side Effects of Trientine (Curior) and Emergency Management

Neurological Deterioration

Risk: During initial treatment, neurological symptoms may worsen due to copper mobilization.

Response: Immediately adjust the dosage or discontinue the medication, and monitor serum non-ceruloplasmin copper (NCC) levels.

Copper/Iron Deficiency

Risk: Long-term medication use may lead to copper or iron deficiency, which can affect fetal development during pregnancy.

Response: Regularly monitor copper/iron levels; pregnant women must have their dosage strictly controlled.

Allergic Reactions

Symptoms: Rash, difficulty breathing, etc.

Precautions for Daily Administration of Trientine (Curior)

Dosage and Administration Method

Initial Dosage: The usual dosage for adults is 750-1500 mg per day, administered orally in 2-4 divided doses.

Adjustment Principle: Adjust the dosage based on the results of NCC and 24-hour urinary copper excretion (UCE); the maximum daily dosage must not exceed 3000 mg.

Drug Interactions

Contraindicated Concurrent Use: Mineral supplements (iron, zinc, calcium, magnesium) — should be taken at an interval of 1-2 hours.

Other oral medications — should be taken at an interval of at least 1 hour.

Administration in High-Risk Populations

Pregnant Women: The risk of copper deficiency and disease control must be balanced; copper levels should be monitored throughout the pregnancy.

Elderly Patients: Start with a low dosage and pay attention to liver and kidney function.

Storage and Administration Details

Storage: Store away from light at 20-25°C; take the medication immediately after removal from storage.

Administration: Swallow the tablets whole; do not chew or dissolve them. For patients with difficulty swallowing, tablets can be split along the score line.
