Delgocitinib cream is a topical prescription drug indicated for adults with moderate-to-severe chronic hand eczema. Clarifying its target population and correct usage is a prerequisite for ensuring efficacy and safety.

I. Indications and Target Population

1. Indication

(1) Delgocitinib cream is indicated for the topical treatment of moderate-to-severe chronic hand eczema in adults.

(2) Chronic hand eczema is characterized by recurrent dryness, erythema, scaling, hyperkeratosis, or vesicles on the hands, accompanied by marked pruritus or pain, with a prolonged disease course.

2. Target Population

Mainly for two groups of patients: those with an inadequate response to topical glucocorticoids; and those in whom topical glucocorticoids are unsuitable due to adverse reactions or contraindications.

3. Non-target Population

(1) Safety and efficacy in children have not been established; use in pediatric patients is not recommended.

(2) Use should be suspended during active severe infections.

(3) Pregnant and breastfeeding women should consult a physician before use and weigh the benefits against risks.

4. Combination Therapy Restrictions

Concomitant use of this product with other JAK inhibitors or potent immunosuppressants (e.g., cyclosporine, azathioprine) is not recommended to avoid excessive immunosuppression.

II. Recommended Dosage and Administration

1. Maximum Amount

(1) Total use should not exceed 30 g per two weeks, and not exceed 60 g per month.

(2) Exceeding these limits may increase the risk of systemic absorption; strict adherence is required.

2. Frequency and Site of Application

(1) Apply twice daily (morning and evening), only to the affected skin areas on the hands and wrists.

(2) Apply a thin layer evenly to cover the lesions, gently rub in until absorbed; do not apply thickly.

3. Pre-application Preparation

Wash hands and affected skin with a mild cleanser before application, and pat dry gently with a clean towel to ensure the skin is clean and dry before applying the cream.

4. For External Use Only

(1) This product is for cutaneous use only. Oral ingestion is strictly prohibited. Do not apply to the eyes, mouth, vagina, or other mucous membranes.

(2) If accidental contact with mucous membranes occurs, rinse immediately with plenty of water.

III. Important Precautions During Treatment

1. Complete Vaccinations Before Treatment

(1) Before initiating treatment, all age-appropriate vaccinations should be completed according to current immunization guidelines, especially the herpes zoster vaccine.

(2) During treatment and shortly after discontinuation, avoid receiving live vaccines.

2. Avoid Sun Exposure

(1) There may be a risk of non-melanoma skin cancer during use.

(2) Limit sun exposure; wear protective clothing or use a broad-spectrum sunscreen when outdoors, and avoid using tanning lamps.

3. Watch for Signs of Infection

(1) This product may mildly reduce local immune defense. If fever, chills, skin redness, warmth, pain, or vesicular rash occur during treatment, seek medical attention promptly.

(2) If a serious infection occurs, suspend use until the infection is controlled.

4. Special Populations

(1) Breastfeeding women should avoid contact of the medicated hand skin (especially around the nipples) with the infant after application, to minimize drug exposure.

(2) No dose adjustment is required for elderly patients, but they should still follow their physician's instructions.
